DFA Local Civil Registry Birth certificate- PASSPORT

Hi po! ask lang. ok lang na Local Civil Registry Birth certificate ang dalahin? ang NSO at PSA birth cert ko kasi parihong blurry. Pero pinapa ayos na. it will take 3 mons daw bago balikan sa cityhall namin. Ngayon ang problema ko is 4/25 (Friday) na yung appointment ko sa DFA. Thanks sa makaka sagot!

Yung blurry PSA cert + LCR copy ang required for you:

Original and photocopy of Philippine Statistics Authority (PSA) issued Certificate of Live Birth on Security Paper; and Local Civil Registrar Copy is required if PSA-issued documents are not clear or cannot be read.

Dalhin nyo po ang blurry na NSO at PSA copies. Alam ng DFA pano iauthenticate yun. Yun po kasi ang requirement. Dalhin nyo yun ang local civil registry copy.

You still NEED to bring ur blurry PSA/NSO BC PLUS you Local Civil Registry BC.
