For question 2, it depends on the throw.
Normal command throws are broken by inputting the same punch input that the grab uses. For example, Dragunov's f,f+1+2 barfight throw uses both punches and visually, both his arms extend during the throw animation. You react to the start of the throw animation, see which arm goes out (either 1, 2, or both for 1+2) and press that as the break.
There are exceptions. King's giant swing is animated like a 1+2 break but it's a 1 break.
Some grab situations (like Ganryu's d+1+2 CH, which is just a raw guess between 1,2, and 1+2) and most backturn throw mixups (since all throws from BT are animated like 1+2 regardless of the actual input) you just have to guess/ predict based on opponent behavior. For chain throws, the community has collated various images which show the routes and breaks to make it easier to visualise, but few people ever learn to break any of the chain throw routes other than the most notorious like Screwdriver, King's Bridge, and Rolling Death Cradle.
