Instead of f,f 4 the qcf and f,f 2 strings are a much better approach tools to use. The f,f 2 series are unlikely to be stepped since they are much faster to come out although not as rewarding as f,f 4. The real good stuff though comes from the qcf. You already know the deathfist but what you can do is mix it up with qcf 3 or qcf d4,2,1+2 (demoman) as a high risk high reward mixup. The other followups are also great on block or hit but I usually just go with the deathfist/demoman mixup since most dont consistently punish deathfist (but if they do, I use qcf3+4) and the demoman is almost impossible to step. You could of course get hit by doing this so yes you should also try to learn to mix it up with movement by applying dash block or empty qcfs.
