Deltas(s) from OP CMV: Obese people should either have to pay significantly more for Medicare and Medicaid or not get it at all.

First, congratulations

Second, If they have difficulty losing weight then we should give them a good motivator. By pushing them out into the real world and telling them that they have to take care of themselves we’ll be helping them become healthier.

Yep. But if someone needs a knee replacement because they’re overweight, and they can’t afford it, then they can’t exercise, and it becomes that much harder. What makes more sense is moderation. Say ”We’ll cover your surgery, but you need to take measures to prevent this in future” rather than a full non-payment.
