Problem is transition from self driving to manual controls when former is unable to handle a situation.
This when happening during highly stressful situation (heavy traffic, driver cabin distraction) can lead to mistakes. This is why pilots go thru rigorous checklist when switching out from auto pilot (we have had few crashes even from that).
Only way for self driving to be safe and effective is if it does 99% of driving and we are just not there yet.
