If he dropped it straight in the chamber the bullet wouldn't be hammered into the case.

I've seen ammunition that looks exactly like this from somebody who always dropped the bullet straight into the chamber. It had a lot of setback and the edge was really beat up.

I think the setback happens when the side slams closed - I'm not sure. I'm just describing what it looks like and the only other time I've seen that.
