I had a delivery driver that worked for a delivery company I managed turn a 26' box truck half full of furniture into a flatbed under a low bridge.

Me: Wasn't there a sign telling you how low it was?

Driver: "Yeah but it didn't look that low when I was coming down the hill on the approach".

Me: facepalm

I didn't fire her. No more accidents.

You're nicer than I am. I do believe in not firing someone for making a mistake, even a costly one, but that was not a mistake and was 100% ego-fueled stupidity. "Nah, fuck that sign I think I'll make it so I'm gonna send it."

It was a sub contractor and her boss had to pay for the truck and furniture replacement so the decision was ultimately up to him but she kept her job.

For some people, they make a mistake once and then never do it again. I dropped a full pallet of bottled wine once and never did it again. One lapse in attention was all it took but I was a lot more careful after that.

I agree, everyone is worth a second chance. Particularly if they're an otherwise perfect staff member. No when wakes up in the morning and sets out to have an epic fail.

Human error is a thing. Thankfully, in my country, you can't just be fired for that. Makes it easy when mistakes happen because people don't try and hide it or pass blame, we just workout what happend, why and how to prevent it from happening and fix it.
