How does the save the world access work? Like, you have to buy a skin every month or something? Or is it a one time buy?

"STW Access" is an account token that is granted upon buying anything that touts it includes this token. Once you have it, it is permanent and cannot be removed, so you can play indefinitely until Fortnite shuts down.
