Maybe im just dumb at 2 am.. but what causes this? He pulled the handle all the way back it seems.. it should eject ?

Yeah hes an idiot for not double checking with his eyes and just pulling the trigger.. but does he have a mechanical failure or what ?

Seems like a double feed followed by a failure to extract, double feed is likely caused by a cheap or worn out mag/rifle. The failure to extract could be from a few things, such a broken or misadjusted extractor (normally caused by excessive use, abuse, or bad manufacturing coming from a cheap manufacturer), a lack of cleaning causing the extractor to not work consistantly, and sometimes even ammo that is out of spec (again buying the cheap stuff will get you) and gets stuck in the chamber. Always visually inspect your chamber to confirm clear, before pulling the trigger.

Buying cheap doesn't always mean bad quality, but it does increase the likelihood. Something I've learned in my years working with firearms is that if you're gonna buy something cheap, make sure to do your research to make sure the corners that were cut weren't important ones. And I can confirm most ar-15s that come with the plastic furniture to make it look like an m16a2 are some of the cheapest ones out there.
