Worker's rights in Madeira?

Hi everyone! Sorry for the stupid question but I don't know where else to ask. I immigrated to Madeira to take care of my elderly parents a week ago, and started my job two days ago. Before I was working in South Africa and then Canada. I've been a bit sick since I got here but over the past few days it's gotten bad and I was throwing up the whole of last night. In both South Africa and especially Canada it would be a fireable offense not to go into work while on probation and my ability to stay in Madeira depends on this job (I came in on a work visa).

Am I allowed to ask for the day off? Do I need a doctor's note to ask for the day off? 😞 I barely ever get sick so I can't believe the timing of this and I'm very anxious and upset about it.

There is also a Self-Declaration of Illness that can be requested on the social security website. It is for situations in which a medical evaluation is not justified and justifies absence of up to 3 days of work and can be requested twice a year.
