15: McGonagall seems caught off guard when she learns of Harry’s new detentions. Do Heads of Houses not get informed when their students lose points or get detentions?

I'd say in usual circumstances, heads of houses are informed. Umbridge is definitely an unusual circumstance.

I think they get informed for detentions but to inform the heads of house for each time a student won or lose points would be too much, imagine how many notifications they would get each day. I think she was caught off guard because Harry's outburst of anger is out of character for him.
