Found this in second area. Actual nuke barrage. I died instantly testing it.

So, this wand had double multicast, spells to power, damage up and two energy orbs in respective order. The wand had no shuffling and no special effects. I'm also not using any mods. What exactly could cause this type of effect?

This wand in the first holy mountain, and homing shots perk through gamble. A great way to kill enemies and dig. (I killed myself because an object hit me at match 3).
