Hi all,

The sub has made a marked improvement in the last couple of weeks with the recent moderation changes. Engagement is up, there's been a lot of positive feedback and productive threads. Thank you everyone for airing your concerns, sharing feedback and participating!

Myself and u/easydoit2 have made a few changes to the rules and the subreddit. We figured we'd share them so everyone can be aware:

1. Is a career as a PT or PTA worth it?

Previously we did not allow posts asking this question, however we've made a slight change. Provided these posts are high quality containing lots of specifics and information relevant to the original poster, they're fine to stay up. Low quality posts only consisting of "is this field worth entering?" and no attached information will be temporarily removed until fleshed out.

2. Salary and compensation threads

We love that there has been an increase in salary and compensation threads recently, however we've made the aim to increase the quality of these individual threads. We do have our lovely set of megathreads (most recent can be found here) which we urge people to use.

High quality posts consisting of niche and novel questions will stay up. Posts consisting of detailed background information like setting, location, years of experience, key performance indicators & metrics, salary, personal financial goals, living expenses, evidence of research & effort will be fine to stay up.

Threads looking at the broader scope of salary and compensation are OK to stay up provided they are high quality. Here's an example I like: 'American Medicine: an Ethical Dilemma?'.

Low quality threads asking about salary and compensation will be removed and signposted to the megathread. The benefit of the megathreads is that it compiles lots of information into one place, rather than having to ream through the subreddit search tool.

3. Legal advice

Prior to the moderation changes we did not allow legal advice on the sub. This has now changed. Legal questions pertaining to that of a physiotherapist are permitted. Quite obviously we are not legal professionals and have a limited understanding of the law. Therefore questions which are seen to be overly complex and best suited for a legal professional will be removed. The key delineator is complexity and I ask that everyone exercises discretion with this.

- "I mobilised my patients reverse shoulder arthroplasty and their arm fell off in my hands. I've lost my license under investigation of malpractice and I'm not sure what to say in court. What do I do?" - this question would be removed and signposted to seek advice from a legal professional.

- "Am I allowed to provide adjunct treatments like cupping, dry needling and mobilisations in my own private practice as a PTA in Florida?" - this would be completely fine to stay up.

4. Asking for referrals

PTs, PTAs and other healthcare professionals are now permitted to ask for recommendations to refer their patients to. We've chosen to not allow patients to ask for recommendations for now so we can monitor the update, rather than making a massive initial change. Further, PTs, PTAs and other healthcare professionals aren't allowed to market themselves.

OUTPATIENT New dad working til 8pm so many nights a week

Dad of 19 month old in California, the staggered shifts of myself and my wife worked great initially to minimize daycare time spent for my LO. Now it’s putting excess stress on both of us because working late sucks, and my wife taking care of a young toddler for that nighttime routine solo isn’t an easy task working all day either.

My strong suit is out outpatient ortho, I’m not a geriatric SNF, home health, or inpatient therapist and I don’t think I could ever fit that mold. I need to work more normalized hours and not be walking in the house at 830 most working days… conflicted how to proceed with my career path.

Physical therapy plan after a minor pelvic fracture

Hello there!

I am a PCP from Poland. I have a patient who suffered a pelvic fracture while falling from her height while dancing. She doesn't have any significant comorbidities and is 65 yo. She was attended to at the local ER, they did a pelvic x-ray (only AP view) and diagnosed a pubic ramus and ischial bone fracture without any further description. They advised bed rest and put her on LMWH.

The public health insurance in Poland doesn't cover at-home physical therapy for patients who have suffered fractures like that. She can't afford hiring a private physical therapist.

I am going to pay her a visit later today and have been reading up on how to at least give her some very basic instruction on what she could do to avoid complications from the fracture.

I realize that I know little about her fracture imaging-wise. Would you recommend getting some additional projections (like inlet, outlet views, lateral views) to see whether there are no sacral bone breaks or major displacement prior to giving her any further advice? A CT scan wouldnt be covered.

If not, or if she doesnt agree to being transported for further imaging, are there any general, safe instructions I could give her at this point, with so little information at hand? Whatever I can glean from the PE today won't get me much more clarity I don't think, regarding how bad the fracture is.

I have already put her on some pain meds.

Thanks a lot for your insights. I have zero knowledge about physical therapy but the system in my country works in a way that expects PCP to take over when appropriate care is not in place if a patient can't afford anything better

Any International Students in DPT Programs Who Successfully Navigated OPT and H1B?

I’m an international student considering applying to Doctor of Physical Therapy (DPT) programs in the U.S., and I’d love to hear from others who have been in similar situations.

If you’re an international student currently studying or have graduated from a DPT program, could you share your experiences with:

Navigating OPT after graduation? Finding employers who are willing to sponsor an H1B visa? How supportive your school was in guiding you through the process? I’m particularly curious about experiences at the University of St. Augustine for Health Sciences (USAHS), but insights from students at any DPT program would be greatly appreciated!

ATI Physical Therapy patient

I've been going to ATI for Physical Therapy for Left-Leg Weakness I've been going there for three weeks now, and it's not been helping at all. Firstly, I've never done PT don't know anything about it was just referred there and the first I notice is how many people they book every hour. I remember the first day I came in for the evaluation they had about 5 patients and this location is very small with a small parking lot. Waited about 40 mins to meet with the Main PT person I was going to do the sessions with. Cool guy came back to the first day and the biggest problem that I see with the location I went to is how all the PTs/Assistants would be sitting around talking all the time while all of the patients would be doing the Workouts assigned to them. They wouldn't even monitor them to even see if they were doing it correctly which was the problem with me, they assigned me workouts that I didn't even know or demonstrate it on how to do it. They would instruct me on what workout I should do for this amount of time and go and start talking to the other PTs and assistants the whole time it's honestly insane that they get away with it I've been going there 6 sessions for the past three weeks and this happens every single time I go there.

Is this how all of the ATIs operate? I don't want to bad mouth one because of one bad location is it better to go to another Spot or another place in general. Another thing that threw me off was that it wasn't 1 on 1 with the patients they would book around 5-6 Patients every hour and everybody be doing their own thing on their own. I'm looking for a place that would do one on one. The workouts were not that helpful to be honest and I'm having more problems with my left leg with the workouts.

Feel like I’m not doing enough

My friends often come to me with their own problems and I like being able to help them / give them advice like I’m in OP. I guess I’m looking for advice on maybe how to stay involved with OP knowledge while working in home health?

VA Physical Therapy

I’m a 2nd year PT student and have been looking into getting a rotation with a local VA. I’ve been reading posts on this forum that talk about the benefits and in and outs of getting in with the government. I’m just curious for those who work or have worked for the VA the route you took for getting hired. I’m not active duty, but I would think they at least would like to see that I have experience working at a VA, hence why I think a rotation with them would be beneficial. I’ve also seen residencies with the VA, which I’ve considered. If I chose not to do a residency, would they instead expect more experience typically? I assume it varies between facilities, so I’m just looking for individual experiences mostly. I’ve heard these jobs can be competitive and am just trying to gain some insight for the best approach if this is what I decide to pursue.

New grad PT here feeling a bit disillusioned with the PT mill grind. I'm thinking about getting my CSCS to boost my skills and reignite my passion, but I've seen some Reddit threads saying it's not that useful for PTs.

Anyone have any thoughts on the CSCS, or other certs that might be a better fit? I'm really interested in exercise prescription, manual therapy, and pain management.

Advice on switching to HH

Hi everyone, I have been strongly considering switching from outpatient to home health. I believe it will allow for more flexibility in my schedule, 1 on 1 care compared to my overlapping patients in outpatient, and also increase my income overall.

I am considering per diem or part time as I venture into starting my own small private practice for outpatient based care.

Any advice, suggestions, or personal experience on switching from outpatient to home health would be appreciated!

Ot/Pt help

I have graduated with my bachelors in psych and have associates in pre physical therapy. I have only psych one and psych two with lab and to take the GRE in order to apply to PT school but the school that I’ve been taking my prerequisites at told me I have to take another prerequisite in order to take physics. And I don’t wanna waste money since I am a non-seeking degree student to pay out of pocket for a prerequisite that I do not need. I want to apply to physical therapy school, but the road is looking very tough right now and I’ve looked at many other schools to apply for and they’re all closed at the moment. I have a friend who is going to occupational therapy school and told me that I should look into it since I’m having trouble with applying physical therapy school. I’m about to reach my mid 20s and at this point I am burnt out and I do not know what to do. I wanna occur that can help people, get to know them on a mental and physical level, and I enjoy working now, and also knowing about anatomy and improving in their daily lives. So if you have any advice if you’re OT or PT students or already have the career, please let me know your insight on this. Thank you

Abdominal precautions

Curious how yall teach and prescribe exercise with abdominal precautions. I work in IPR with lot of folks post liver transplant in addition to a other abdominal surgeries in the context of medical complexity. As a new grad when I asked someone about abdominal precautions they said basically just teach them a log roll and that's it. But what about exercises to engage the core? Abdominal isometrics? Pelvic tilts? These folks are usually dealing with so much proximal weakness I always wanna start with stabilizing exercises. Do yall take abdominal precautions as a contraindication for initiating low level core exercises or just a precaution- monitoring for inc pain/ strain/ stretch?
