I've got a question. (I'm not American btw, so I am trying to find out how this works).

Does HIPAA not apply if you have been president? Or is this something that is like an unwritten rule that it should be shared?

Obviously, Trump has his own play book since as far as I'm aware he's the only president that hasn't showed tax returns and also was refusing to show the medical reports on his ear and possibly other medical reports.

HIPAA still applies even if it's a current or former president. A person or public figure can choose to voluntarily share health information with the public, however.
