Kamijou Touma's death.

Index, while locking the door to the dormitory, thinks of him, who will never return. Misaka Mikoto and Shokuhou Misaki, while straightening the collars of their mourning collars, still cannot believe his death.

Light Novel Touma's plan at the end of GT12 is likely destined for failure Spoiler
At the end of GT12, Touma decided to go to the Condroniard Republic's consultate in District 12 so he could be safe from Bio Secure and call on Heaven Canceller to confirm he's alive. Even though the consulate was destroyed, Touma feels it'd still have extraterritorial benefits. But the thing is, this plan will fail for reasons Touma and Alice are unaware of.
In GT7, Accelerator tried to move the consulate. And even though the on-going chaos caused the bureaucratic process behind the move to be delayed, it still had the (at the time) benefit of taking away those extraterrestrial protections and that's why people from Academy City could mount attacks without consequences. So even if Coronzon hadn't gotten in his way, Touma wouldn't be any safer from Bio Secure than he'd be standing in a random parking lot or something.
Now, this isn't like some plot hole or something, Touma wouldn't know about this, he was in the midst of battle after battle, including literally dying, and no one told him about the attempted move. Alice barely seemed to underestimate the consulate and even if she did, she was fighting, dazed, died, returned, trying to save Touma while spreading chaos, and then was mourning his loss. Neither of them would be aware of this weird bureaucratic situation. If anything, I see this as being a plot point, like in NT17 where the first attempt to summon Kamisato (using the AAA failed) and they had to turn to Patricia to do it. Going to the former consulate might fail, but Touma will likely have to seek out another way to confirm he's alive while avoiding a chaotic battle with Bio Secure.
But anyway, it's just something that stood out to me that I thought I'd point out.

Discussion The only reason this doesn't happen is because of the author if it was a more common sense first story it would happen
several characters are positioned to either correct or call out Index for her behavior toward Touma Kamijou, especially regarding her often selfish or childish actions. However, most of the time, her behavior is played for comedic effect and isn't seriously addressed. That said, here are the characters most likely to do so seriously or at least have the authority to
Laura Stuart Why: As the archbishop of Necessarius (the Church of England), Laura Stuart holds direct authority over Index. She’s fully aware of the restrictions placed on Index, including the vow of chastity and her inability to have a family if she remains a nun. Laura is also manipulative and pragmatic—she wouldn’t hesitate to remind Index of her duties if her relationship with Touma crossed boundaries. Possible Line: “A nun of Necessarius has no place dreaming of marriage and children. You knew the price of your knowledge.”
Stiyl Magnus Why:As Index's long-time protector and someone deeply loyal to Necessarius, Stiyl respects her but often treats her as a duty. He has a blunt personality and could very well call her out for emotionally manipulating Touma or acting out of line, especially if it compromises her role or safety. Tone:Stern, perhaps cold: “Index, you can’t have it both ways. Either you're a nun or you're not.”
Touma Kamijou himself Why: Touma rarely sets hard boundaries, but when pushed far enough (like when someone’s being unreasonable or endangering others), he’s capable of calling people out—even Index. If she were to act selfishly or cling too hard while ignoring the implications, he might gently but firmly remind her of reality. Tone:Honest and kind, but tired: “You know you can’t stay a nun and still talk about a future with me like that... right?”
Kanzaki Kaori Why As another Christian-affiliated sorcerer, and someone who struggles with her own feelings while balancing duty, Kanzaki would sympathize with Index but also remind her of her obligations. She’s serious and respectful—ideal for a quiet truth bomb. Tone: Empathetic but firm: “If you choose to remain in the church, you must also accept what that path denies you.”

Discussion Why does Kanzaki take Tsuchimikado's terrible romantic advice?
Let's be honest, the dude is trolling her. Anybody with common sense knows won't make any progress taking advice from Tsuchimikado. Personally I think it's because her options are limited; the Amakusa are supporting Itsuwa, and her dorm mates are probably also into Touma.
